Exciting Office Update! We are now seeing patients in our newly remodeled locations at 330 South Garden Way Eugene, OR 97401. Dr. Hoffman is seeing patients out of suite 150 and Dr. Sims and Dr. Velazquez are seeing patients out of suite 120.
You will find our new optical shop in suite 120 with a huge selection latest in frame styles and lenses to fit every budget. We still have our Florence office, but we no longer have offices at Oak St or Harlow Road, please call our office with any questions, 541-687-2110.
We are a full service ophthalmology practice in Eugene, Oregon, specializing in cataract and refractive surgery including iLASIK, PRK, ICL and Refractive Lens Exchange. Our physicians are subspecialty fellowship-trained in glaucoma and cornea and external diseases, and offer participation in clinical research studies.
Our physicians diagnose and treat nearly all eye conditions including cataracts, corneal diseases, glaucoma, diabetic retinopathy, macular degeneration, double vision, strabismus and amblyopia, eyelid diseases, dry eye, nearsightedness and farsightedness, astigmatism and presbyopia. We also offer comprehensive eye exams, glasses and contact lenses.

With extensive training and experience in refractive surgery, cornea and external diseases, and cataract surgery. Dr. Hoffman is a Certified Physician Investigator.
Dr. Sims is a skilled, fellowship trained glaucoma specialist providing advanced glaucoma procedures, with expertise in cataract and refractive surgeries.
Ulysses Velazquez is certified by the National Board of Examiners in Optometry and excited to provide comprehensive eye care for patients of all ages in the community.
